Yellow fever clinical pathology characteristics and management
r FEVE yellow is an acute arboviral (arthropod) infection caused by yellow fever virus, a disease flavivirus.The characterized by high fever, jaundice andencephalopathy in its severe form. The disease causes outbreaks INFOCAL in Africa, the Caribbean and Central America andSouth. This condition is prevented by the vaccine strain 17Dattenuated live totravellers to be given to endemic areas and outbreaks populationsin Indians. There are rare occurrences of yellow fever importationof to Europe, which can present ASPU or viral hemorrhagic fever.
Distribution and incidence tropical sub-Saharan Africa (especially West Africa), theCaribbean islands (most recently Trinidad) and South America (Brazil, Peru, Bolivia, Ecuador, Colombia and Venezuela) are the areas suffering from yellow fever. Relatively small number of cases occur each year in theseareas, although cases isolated areas may go unreported.
Transmission and epidemiology in South America and central and eastern Africa, a sylvatic cycle implies monkey-mosquito-monkey and keeps the virus in the reservoir themosquito. Haemagogus mosquitoes are thevector. The epidemiology in the savanna and forests implies savannahareas andmonkeys Aedes other human beings. There Mayth be a short period of remission of symptoms to24 hours before recurrence of fever with jaundice vomitingand. Bleeding in the intestine, skin and other sitesis common in severe cases. Occasional cases are seen limited organdamage heart or kidneys, orrenal causing heart failure. Bleeding and renal failure are the main causesof high mortality (up to 50%) in severe cases. Severe cases can Resolutionin take 3 days to 6 weeks.
Diagnosis Clinical features in severe cases suggests the diagnosis, but in cases milder, with no evidence of organ dysfunctionthe diagnosis can not be done. The virus can be isolated from the blood mostoften taken in the first four days of antibodies illness.Rising degrees in clinical sera may also give the diagnosis, but serological tests may not be easy to interpret inpeople exposed to related viruses.
The differential diagnosis in mild cases of febrile illness range is extensive and includes consideredis malaria, typhoid, prodromalphase viral hepatitis, leptospirosis and rickettsialdiseases. Coagulationabnormalities include prolonged prothrombin time, reduced levels of fibrinogen and fibrin degradationproducts detectable. Renal failure with proteinuria, oliguria, creatinine andraised and urea can be produced.
Prevention and control of all travelers to endemic areas - apart from pregnant women, children under 1 immunosuppressedpatients year - should receive fevervaccine attenuated 17D yellow. Patients should be nursed yellow fever undermosquito networks to prevent mosquitoes from using infected.Vaccination increasingly to help control epidemics.
